Home Telehealth Program

STVHCS’ Home Telehealth Program places electronic messaging systems in chronically ill patients’ homes to monitor symptoms, blood glucose, blood pressure, or weight changes which play an important part in managing chronic diseases.  We also provide TeleMOVE services to overweight Veterans who are interested in losing weight. Veterans respond to questions and submit measurements by phone each day.  Responses are transmitted to a web site where they can be reviewed by an RN Care Coordinator who works closely with the Veteran’s PACT team with the goal of intervening early to prevent serious problems or hospitalizations.  There is no cost to participating Veterans.  Hours of operation are Monday through Friday, 8 a. m. - 4:30 p.m. except on holidays.  The Home Telehealth program is located in the One Technology building about one block from the VA hospital off of Babcock Road.  If you are interested, please contact us at (210)616-8256 or (210)616-8271.
