Research Privileges VA NTE/Salaried Human/Animal/Lab, Human/Animal, Human/Lab Research

Thank you for seeking to continue your involvement in the WOC program.
These documents are for extending your WOC for the purpose of Human/Animal/Lab Research, Human/Animal, or Human/Lab research.
Please complete and return documents listed below in person or electronically(encrypted/secured)to start the process.
If you can no longer access your TMS account please contact me so that I can set it up and assign the proper courses.
At 90 days prior to the expiration of your appointment you should be working on this paperwork.
You should have the below paperwork submitted prior to 60 days from your WOC appointment expiration date.

Complete the following:

• Checklist (applicant signs)
• Pertinent 10-2850 form (applicant signs pages 3 & 4)
  • 10-2850 for Physicians
  • 10-2850a for Nurses
  • 10-2850c for all others
• 306 Form (applicant signs 17a)
• I-9 Form (applicant signs page 1)
• IPA Form (applicant signs)

These must be completed/signed by the PI:

• Orientation and Training Form (PI must complete and sign)
• Scope of Work (for Animal and/or Lab only)
  and/or
• Scope of Practice

Please Provide:

• Copy of CV/Resume
• Copy of completion of TB skin Test. (2 required on file. WOC request packet may proceed to HR pending the 2^nd skin test)

Required Training: (if you do not have any accounts created, I will have to do this with you when we meet)

• CITI  - https://about.citiprogram.org/en/homepage/  -(you must ensure to affiliate with VA San Antonio. I can assist over the phone or in my office)
  •  Citi Affiliation Animal Walkthrough
  •  Citi Affiliation Human Walkthrough
• TMS  - will be required to have the following completed or current within 1 year:
  • HIPAA (10203)
  • Security Awareness (10176)
  • Bloodborne Pathogen (NFED 3857349)
  • Tuberculosis (NFED 3867461)
  • Technology Transfer (33534)

First Time Users:

• New TMS Instructions. The new TMS 2.0 updates made changes to how users may log in. (PIV or One-Time Passcode)

  • Please use this website: https://www.tms.va.gov/SecureAuth35/
  • First time users will select "PASSWORD LOGIN FOR NEW STAFF"
  • Enter your USername = "your email address"
  • Enter your password =NewTMSPW#123
  • TMS is not compatible with:MAC, Google Chrome, or Mozilla Firefox.

To Log in:

• New TMS Instructions. The new TMS 2.0 updates made changes to how users may log in. (PIV or One-Time Passcode)

  • Please use this website: https://www.tms.va.gov/SecureAuth35/
  • TMS will no longer use PASSWORDS.
  • Users may use their PIV Cards (select SSO Login).
  • Or may log in with a One-Time PASSCODE. For a PASSCODE... use "Please enter your username below:"   (must be the current email address used in your TMS profile)
  • TMS is not compatible with:MAC, Google Chrome, or Mozilla Firefox

Once ALL requirements/documents are completed, please schedule and appointment to meet with us; provide 2 available time/dates that work best for you.
We are located on the 2^nd floor.
Exiting the main elevators, turn right. You will find a set of brown double doors, immediately next to the men's restroom (says Research at top).
