New Personnel Requiring Research Privileges for Human Studies Must Complete All Items under the Appropriate Category below

VA Salaried, U.S. Licensed Physicians

1.  Documentation of "VA Human Subjects Protection and Good Clinical Practices" training and VA Data Security Training (All 2 Modules)  (NOTE: 2 Modules are VA Privacy and Information Security Awareness and Rules of Behavior and Privacy and HIPAA Training)   
2.  Research Financial Conflict of Interest Statement Form 

Non VA Salaried, U.S. Licensed Physicians (i.e. WOC, Contracts, Fee Base)

1. Documentation of VA Human Subjects Protection and Good Clinical Practices training and VA Data Security Training (All 2 Modules) 
2. Medical Staff Member Appointment Request Form to add Research Privileges to current credentialing status  
3. Signed Intellectual Property Agreement (IPA)  
4. Research Financial Conflict of Interest Statement Form 

Fellows, Med Students, and Residents (Processed through Education Service)

1. Documentation of "VA Human Subjects Protection and Good Clinical Practices" training and VA Data Security Training (All 2 Modules) 
2. Copy of  appointment memo (obtained from the VA Education Office x.15109)
3. Signed Intellectual Property Agreement (IPA)   
4. STVHCS Scope of Practice for Non-licensed Research Personnel 
5. Research Financial Conflict of Interest Statement Form 

Other*

1. Documentation of "VA Human Subjects Protection and Good Clinical Practices" training and VA Data Security Training (All 2 Modules) 
2. Signed Written Release of information ONLY for unlicensed personnel involved in research with educational degree
3. STVHCS Scope of Practice for Non-licensed Research Personnel 
4. Research Financial Conflict of Interest Statement Form 
5. If NOT VA salaried, WOC appointment for non-VA Staff Conducting Research  

*Includes non-physician licensed and/or credentialed providers/professionals (i.e.   Optometrists, Dentists, Psychologists, Pharmacists, Respiratory Therapists, Podiatrists) AND unlicensed physicians with medical degrees  and other unlicensed personnel involved in research

Exempt**

1. Documentation of Development Personnel training (see VA Data Security Training) 
2. All IPA employees: 2 additional modules -VA Privacy and Information Security Awareness and Rules of Behavior, Privacy and HIPAA Training.
3. Verification from PI denoting an exempt role in the study
4. Research Financial Conflict of Interest Statement Form 

**Exempt personnel consists of persons who are not utilizing VA resources (e.g. salary, space, equipment) as part of research project AND only analyze coded data or speciments, or consult on projects without intervention/interacting with subjects or obtaining subject's identifiable private information for research purposes (Note: exempt personnel must check-in as a visitor with the Research Service when on VA property if they do not have a current VA affiliation).

On-Site:

1. Documentation of "VA ORD Biosecurity" training and VA Data Security Training (All 2 Modules) (NOTE: 2 Modules are VA Privacy and Information Security Awareness and Rules of Behavior, Privacy and HIPAA Training) 
2. Research Financial Conflict of Interest Statement Form     
3. Scope of Work for Laboratory Research  
   Also, if NOT VA Salaried, but physically working at the VA:
4. WOC appointment for non-VA Staff Conducting Research 
5. Signed Intellectual Property Agreement (IPA) 

Off-Site (VA-funded):

1. Documentation of CITI training (Biosecurity)     
2. For PIs and all IPA employees: 2 additional modules - VA Privacy and Information Security Awareness and Rules of Behavior,  Privacy and HIPAA Training.    
3. Research Financial Conflict of Interest Statement Form 
4. Scope of Work for Laboratory Research    
5. Signed Intellectual Property Agreement (IPA)  

Off-Site (non VA-funded)*:

1. Memorandum from PI denoting what their role in the study will be and verifying that the individual will not be conducting research on-site (STVHCS).                                                           

*This will consist of persons not physically conducting research on VA property and will not have access to VA sensitive information.
